Instant assessment guide based on anabolic optimal targets and functional medicine safety margins
ALT (GPT)
Primary enzyme leaking into the bloodstream upon hepatocellular injury. Highly sensitive to 17-alpha alkylated oral AAS use.
• Increase NAC to 1,200mg/day + Silymarin (Milk Thistle 500mg)
• Complete abstinence from alcohol and acetaminophen (Tylenol)
• Emergency administration of TUDCA 500–1,000mg/day + NAC 1,800mg
• Immediately halt 17-aa orals and evaluate reducing injectable compound dosages
AST (GOT)
Rises sharply from skeletal muscle damage as well as hepatocytes. Can temporarily spike above 100 post-heavy resistance training.
• Re-test after 48 hours of complete rest from intense training
• Glutathione IV or NAC supplementation
• Concurrent rapid spike with ALT indicates hepatic parenchymal strain → Administer TUDCA 1,000mg
• Run Creatine Kinase (CK) test to rule out rhabdomyolysis
Gamma-GTP (GGT)
Indicator of cholestasis and hepatic detoxification (glutathione depletion). Pure liver-specific marker unaffected by muscle trauma.
• Take NAC 1,200mg to stimulate glutathione synthesis
• Hydration (4L+/day) to prevent biliary stasis
• Risk of cholestatic liver injury → UDCA (Ursofalk) or TUDCA 750mg+ mandatory
• Complete cessation of oral compounds
Total Bilirubin
Reflects red blood cell breakdown metabolism and biliary excretion status. High levels indicate jaundice and cholestasis risk.
• Review hepatoprotective stack if cholestasis is suspected
• Avoid overexertion and fasting
• Potential jaundice onset → Seek immediate medical evaluation and full cycle OFF
Cystatin-C
Gold standard for renal glomerular filtration rate, unaffected by muscle mass or protein intake. Top priority test for bodybuilder kidney assessment.
• Aggressively increase daily water intake to 4–5L
• Strict blood pressure control (target < 120/80 mmHg)
• Active glomerular injury in progress → Immediately drop nephrotoxic compounds (Trenbolone, orals)
• Administer Astragalus extract (3,000mg/day) and consult nephrology
Serum Creatinine
Muscle metabolite. Can elevate up to 1.4–1.6 without renal pathology in high muscle mass or creatine supplement users.
• Temporarily halt creatine supplementation and cross-reference with Cystatin-C
• Maintain hydration during high-protein diets
• Concurrent elevation with Cystatin-C indicates acute kidney injury → Cycle OFF and immediate BP reduction
eGFR (Estimated GFR)
Overall metric of renal filtration capacity. Values below 60 signal chronic kidney function decline.
• Account for creatinine distortion via high muscle mass → Recalculate using Cystatin-C formula
• Persistent values < 60 risk irreversible renal damage → Cease all PEDs and undergo nephrology workup
ApoB (Apolipoprotein B)
Total count of atherogenic particles penetrating arterial walls. Supremely more accurate cardiovascular risk factor than LDL-C.
• Consider Citrus Bergamot 1,000mg/day + Ezetimibe (10mg prescription)
• Restrict saturated fat intake
• Rapid plaque formation risk → Add statin (Rosuvastatin 5–10mg) or immediately drop orals
Hematocrit (HCT)
Red blood cell volume ratio in blood. Excessive erythrocesis from AAS thickens blood, triggering stroke/myocardial infarction risks.
• Daily hydration > 4.5L + Naringin (grapefruit extract) supplementation
• Zone 2 cardio 3-4x/week mandatory
• > 54% spikes thrombosis risk → Immediate therapeutic phlebotomy/blood donation (400–500mL)
• Reduce EPO-stimulating compounds
hs-CRP (High-Sensitivity CRP)
Vascular wall micro-inflammation marker. Reflects endothelial damage and lipid plaque rupture vulnerability.
• High-potency Omega-3 (EPA+DHA 4,000mg/day) + Curcumin supplementation
• Treat periodontal and joint inflammation
• Acute cardiovascular event hazard → Low-dose aspirin (100mg) and vascular workup
HDL Cholesterol
Vascular scavenger role. Frequently crashes below 10 due to hepatic lipase stimulation from oral 17-aa AAS use.
• Niacin (500mg) + high-intensity cardio regimen
• Increase Omega-3 and extra virgin olive oil intake
• Reverse cholesterol transport paralysis → Strictly limit oral cycles to 4-6 weeks max then OFF
Sensitive Estradiol (Sensitive E2)
LC-MS/MS assay mandatory. Critical balance for gynecomastia prevention, water retention, cognitive function, and endothelial protection.
• 40–50 pg/mL: Fine-tune Aromasin (Exemestane) 12.5mg 1-2x/week
• Crash below 20 requires immediate AI cessation
• Spikes > 60 with breast lump/tenderness → Immediately add Nolvadex 20mg to block receptors
Free Testosterone
Unbound fraction acting directly on muscle cell androgen receptors without SHBG restriction.
• Cross-check Free T to Total T ratio to back-calculate SHBG suppression
• Exceeding target requires concurrent monitoring of erythrocytosis (HCT) and blood pressure
SHBG (Sex Hormone-Binding Globulin)
Hormone transport protein. Hepatic synthesis drops below 5 under potent androgens or oral AAS.
• Adjust AI dosing accounting for simultaneous Free E2 and Free T surges from SHBG depletion
• Prolonged suppression < 5 degrades lipid profile and accelerates endocrine fatigue
Prolactin
Spikes via dopamine receptor stimulation during 19-nor use (Trenbolone, Deca-Durabolin). Causes ED/libido loss and galactorrhea.
• Administer Vitamin B6 (Active P5P form 200–300mg/day)
• Normalize E2 to suppress prolactin receptor hypersensitivity
• Nipple discharge/erectile dysfunction onset → Cabergoline (Dostinex) 0.25mg 1–2x/week
